Long Term Care Ombudsman .

The Long term care Ombudsman is an advocate for residents 60 years and older who live in Nursing Homes and Assisted Living facilities. The Ombudsman has a three fold purpose:
  1. Educate residents, family, facility staff and community about Resident Rights in these settings;
  2. Investigate complaints and concerns regarding care and residents rights
  3. Advocate for Resident Rights in Long-Term Care settings.
If you have a question or concern about resident rights or the quality of care of a resident, please call (435) 752-7242 or 1-877-772-7242 and ask to speak with a local Ombudsman.

If you know of an issue outside of Box Elder, Cache and Rich counties, please contact Adult Protective Services.  They can be reached at 1-800-371-7897.
